Details

No charge for an appointment with a doctor if you have a Quebec health card (CAM). Laboratory fees apply and are refundable through most insurance companies. See the complete Fee Schedule.

You may make an appointment by telephone at (514) 787-0055, at the reception desk, or through the online service.

INSTRUCTIONS FOR YOUR APPOINTMENT WITH THE DOCTOR

  • Arrive 15 minutes before the appointment, to open your file and complete a personal history questionnaire.
  • Bring your health insurance card, which must be valid on the date of the appointment.
  • Plan for laboratory test fees.

  • Plan for a nurse consultation fee.
  • Plan for laboratory test fees.

Window period: Asymptomatic screening (without symptoms)

Please note that a delay no shorter than 3 weeks following risky sexual relations is required to perform screening tests of the principal STI's and a minimum of 2 months for the HIV screening test.

Cancellation policy

We require a minimum 24-hour notice of cancellation; otherwise the deposit will not be reimbursed. To cancel an appointment, access your account or contact us at 514-787-0055.

Reimbursement

When applicable, the deposit is reimbursed by the same method as the payment.

Lateness policy

Clients must arrive 15 minutes before their appointment. Lateness, no matter the cause, may cause you to lose your appointment. It will be up to the health professional to decide whether to see you that day or to schedule a new appointment.

Valid Québec health insurance card

A $100 deposit will be required for patients without a health insurance card or certificate of coverage that is valid on the date of the services, from out-of-province or from another country. The client must pay the physician’s consultation fee according to the fee schedule of the Association des médecins omnipraticiens de Montréal (AMOM), and not according to the fee schedules of the province, employer or insurance company. It is the client’s responsibility to claim for reimbursement afterward.

Communication of results

The results of screening by the nurse will be sent to you electronically to the confidential address that you will have given us. Depending on your situation, a second appointment may be necessary to inform you of the results, such as for the result of an HIV antibody test (conventional test).
